Terms and Conditions

Washington Hospital Healthcare System is pleased to offer our patients on line computer access to portions of their medical record through the WmyChart web portal. If you believe that you would benefit from this service, you may sign up for access by completing an authorization and agreeing to these terms and conditions.

USE OF WmyChart

I understand that WmyChart should NEVER be used for urgent health care matters. The anticipated turnaround time for a response to a secure message is two business days. Therefore, if I believe I am experiencing an urgent or life threatening health condition I should phone my physician, go directly to the Emergency Department of a local hospital or call 911.

If your physician is out of the office or unavailable to respond to your secure email message, your message may be routed to another WHHS provider.

The WmyChart web portal will provide the following services:

  • Ability to review, make or cancel appointments
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information
  • Ability to receive selected lab results
  • Communicate with your physician via secure messaging regarding health concerns
  • Request prescription refills
  • Links to other sites that will provide you health information

Other services may be added as WHHS implements the WmyChart web portal.

I understand that WHHS providers may send me messages via WmyChart. It is my responsibility to monitor these messages and respond in an appropriate manner. I must provide WHHS with a valid email address and update WHHS if I change my email account. WHHS may send me emails via my personal email address to notify me that I have new WmyChart messages. Messages sent via my personal email will not contain personal health information.

I agree that all messages that are sent through WmyChart will be regarding MY health care and conditions. I understand that asking for advice for a friend or family member is not allowed. Since these messages may become a part of my permanent medical record and will be perceived as my health information, thus it is important that no one else's information be reflected in my messages. With my authorization, this information may be released to insurance companies when applying for insurance. I understand that WHHS takes no responsibility for erroneous medical information that I may provide via WmyChart that may impact my ability to be covered by insurance. Without my authorization, medical information may be released to my insurance for payment of my health care claims.

Use of will be governed by California (State) law.

CONFIDENTIALITY

Both Federal and California (State) laws govern the confidentiality of medical information. WHHS is dedicated to keeping my medical information confidential. WHHS affords the same degree of confidentiality to medical information stored on WmyChart as is given to medical information stored by WHHS in any other medium. WHHS is committed to protecting the confidentiality of my medical information. WHHS limits physician and staff access and ability to enter or view information based upon their role in my care. Firewalls, passwords, encryption and audit trails are further used to safeguard my information. All activity on WmyChart is logged and monitored. WHHS employees are subject to strict standards on maintaining your confidentiality. By allowing me access to my medical information you must share this responsibility. PERSONAL IDENTIFICATION CODEUSER ID AND PASSWORD

User IDs and passwords provide authentication and are stored in an encrypted database that is isolated from the internet. As a WmyChart user, my role in maintaining the security of my medical information is to: 1) Change my password on a regular basis, and 2) Keep my login ID and password confidential.

I understand that I will be given a unique password/access code by WHHS to allow me access to WmyChart. The first time I log into WmyChart I will be required to create a unique six character User ID and an eight character password. I agree to keep my user ID and password confidential. Anyone with access to my user ID and password would have access to my personal health information, ability to add to my personal health information and message my providers as if they are me. It is my responsibility to immediately change my password if I believe it has been compromised or shared with someone else. It is recommended that I change my password every 180 days. Every time I log into WmyChart, my user ID and password will be logged in an audit trail in the computer system.

I understand that if WHHS discovers that I have inappropriately shared my password, whether intentionally or unintentionally, or that I have abused my access privileges, WHHS may terminate my participation in WmyChart. I understand that WHHS takes no responsibility for and disclaims any and all liability for damages arising from a breach of medical record confidentiality resulting from my sharing or losing my password.

AVAILABILITY

I understand that WmyChart may not always be available to me due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and I am not guaranteed access at any particular time.

PROXY ACCESS

Proxy access is available for minors and adults. Please contact our Health Information Management Department by calling the hospital's main telephone number (510) 797-1111 and request to be transferred. Someone will be able to assist you with setting up your account.

SECURITY

WHHS has implemented the following security features:

  • WHHS uses recognized best practice policies and procedures to protect the WmyChart and my personal medical information. I can tell when I am secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://, the document comes from a secure server. This means my data cannot be read or deciphered by unauthorized individuals.
  • WHHS afford the same degree of confidentiality to Encryption: We use 128-bit Secure Socket Layer to encrypt the communication from your web browser to our secure web server. All data in WmyChart has been encrypted.
  • Dedicated web server for : We separate the web server and electronic medical record machines. In the unlikely event that our server is compromised, no health information would be exposed since it is not saved on the web server. Patient information is stored only in the electronic medical record, behind a firewall.
  • No Caching on the client side: We disable caching of data in the browser and force the web pages to expire immediately. This prevents a follow-on user from viewing pages that you had previously viewed while using .
  • Firewall: Personal health information is stored in the secure electronic medical record, protected from the Internet by a firewall.
  • Session Timeout: After a period of inactivity, the logon session will time out.
  • Monitoring: We constantly monitor the web server for evidence of unauthorized break-in attempts. We apply the latest security patches, as they become available.
